PD Evaluation Form
After attending an approved professional development event, teachers must submit an online evaluation and and submit proof of attendance within 30 days of attendance. Proof of attendance must be submitted as a PDF or JPG file and may include, but is not limited to, an event certificate, a picture of a name tag with the event's logo on it, or an event handbook with the dates of attendance on it. If none of these are available, you may need to email the event and ask for proof of attendance.
Failure to submit this evaluation will freeze any further requests until the situation is resolved.
